Training/guidance to consumers in performance of ADL’s & community support.

 

Coordinate comprehensive service plan for the qualitative & quantitative aspects of each consumer.  Responsible for the overall daily operation of assigned caseload, oversight of caseload budget, physical plant & staff coordination

Design and coordinate the Health & Wellness programs & initiatives.  Identify trends, assist in implementing programs & make recommendations or changes to existing programs to reflect health & wellness needs of employees & individuals served

HS/GED preferred

 

 

HS/GED, 5 yrs exp working in the Human Services field and 1 yr supervisory exp required.

 

Education/Certification in Physical Education, personal training or health/wellness

CLINIC

Coordinator of Occupational Therapy
Market/40 hrs. per week

 

Physical Theapist
Salary: Market 20hrs. per week

 

 

Provide diagnostic, palliative and rehabilitation services to individuals we serve.  Ensure department meets or exceeds regulatory requirements.  Provide supervision to OT staff.

 

Provide services classified as diagnostic, palliative, preventative and/or rehabilitative/habilitative delivered within the scope of his/her licensure/certification to eligible consumers.  Provide oversight to PTAs

 

Bachelor’s/Master’s Degree in OT with current NYS certification/licensure and minimum of one (1) year supervisory experience.

 

Bachelor’s/Master’s Degree in PT with current NYS certification/licensure required


MAINTENANCE

 

Maintenance Mechanic
$13.20/hr – 40 hrs/per week

 

 

Perform maintenance & repair services to the satisfaction of Programs/Depts as assigned by his/her supervisor.  Maintain neatness & ensure a safe work environment.  On-call responsibility, snow removal

 

HS/GED. Two (2) years in one or more of the following trades:  carpentry, electrical, plumbing, heating, painting, maintenance

Assembly Worker

Provide assistance & supervision to consumers at various work sites.

Provide services at home and in the community to individual with Cerebral Palsy living at home in Forestburgh.  

 

To facilitate & implement recreation interests of program participants with the Recreation Dept and to provide oversight to individuals that are site based after program/work.
